People & Systems is assisting a social service organization located in a suburb north of Boston with their search for a Clinical Case Manager.

As the Clinical Case Manager for this reputable agency, you will work closely with diverse individuals in hopes of transitioning them to safe and affordable life situations by working closely with them to stabilize their unique complex situations. You will provide professional support and insight while working with them to create service plans that addresses their unique needs and providing referrals to other service providers. Your ability to form sincere and solid relationships will allow you to easily and regularly maintain contact with families throughout the process ensuring lasting stability for clients. You will have the opportunity to work as part of a growing team, participate in meetings, maintain up-to-date client files, collect and report on family status, and meet to review your caseload, identify issues/concerns, and receive feedback/support.
QUALIFICATIONS: This position requires a Bachelor's degree (Master's preferred) and 1-3 years of human services experience. Prior experience working with individuals facing addiction, mental illness, or poverty is a plus. A valid driver's license and ability to provide your own transportation are a must. Resiliency, professionalism, a positive outlook, and ability to work closely with diverse individuals will be critical to your success in this role.
