Health & Education Services, Inc.'s Youth Mobile Crisis Intervention Services (YMCIS) programs in Lowell, Lawrence, and North Essex are seeking Family Partners.This position provides support to youth and their families during their involvement in the Youth Mobile Crisis Intervention Services and enables the YMCIS to provide a 2-person response to their many requests.
Qualified candidates must have:
1. Experience as a primary caretaker of a child with Serious Emotional Disturbance, and have experience navigating the service delivery system.
2. The ability to accept youth and their families without judgment or blame for the problems they face.
3. A valid Massachusetts Driver's License.
Bachelor's degree preferred.
Duties:
* Accompany Crisis Clinician during mobile home visits to homes, schools, and other community based programs.
* Check in with clients and family members in order to update them on the status of efforts to check insurance, identify a disposition resource, contract collateral treatment providers, or locate community resources.
* Provide transportation or arrange for transportation for consumers to and from ESP, home setting, or appropriate intervention.
* Accompany clients to appointments in person to provide support, advocacy and active linkage to services and resources (i.e. seek housing, apply for SSI, MassHealth, job interviews, job training, etc.).
* Provide next day follow-up to increase linkages of clients to the acute care referrals and to any ongoing services.
